Services available in Nebraska

HIE

(Health Information Exchange)

CyncHealth provides access to the HIE through a secure method to access health information via a web-based portal and is a CyncHealth core product offering for participants. It enables clinicians to query the longitudinal health records for near real-time patient information, regardless of where patients seek care, enhancing their workflows to deliver safe, effective, high-quality care. The clinical portal is also connected to eHealth exchange, the largest query-based, health information network in the country.

PDMP

(Prescription Drug Monitoring Program)

CyncHealth offers access to a patient’s comprehensive medication history through the PDMP – a stand-alone web-based medication query platform, as medication history viewable in the clinical portal, or as an integration with a participant’s her or pharmacy system to improve workflow for providers. Nebraska was the first state PDMP to require daily dispensing reports of all prescriptions, improving patient care and medication safety, reduce risks associated with polypharmacy, and aiding the prevention of the misuse of controlled substance prescriptions and continues to be a national leader in this space.

ENS

(Event Notification System)

Event Notification Services (ENS) are automated health information alerts from discharging acute and critical access hospitals to a care coordinating provider. This application messages assigned care managers and/or providers when their patients have an event such as Emergency Department (ED) or Inpatient (IP) admissions/discharges. Providers receive notifications on events such as a patient being discharged from a hospital to home or hospital to nursing home and other care transitions.

Quality Reporting

As a CMS Quasi-Qualified Entity and a participant in other quality certification programs, CyncHealth provides a variety of quality reporting services for participants.

Social Care Referral Platform

CyncHealth provides access to our social care referral platform to participating providers at no additional cost, enabling provider to refer patients for social care needs to support better health outcomes. Referrals are supported by a local 2-1-1 Navigator in partnership with United Way to ensure higher rates of fulfilled referrals. Providers may refer patients for several services, including but are not limited to services for housing, food, utility assistance, and transportation.

CyncHealth also provides the Protocol for Responding to and Assessing Patient’s Assets, Risks and Experience (PRAPARE) for social needs screening. PRAPARE has been translated into 26 languages and was vetted for cultural appropriateness to help reduce barriers to social care.

HISP Direct Secure Messaging

As a HISP, CyncHealth provides Direct Secure Messaging utilizes the Direct Standard™, a technical standard for exchanging health information between health care entities (e.g., primary care physicians, specialists, hospitals, clinical labs) for sending messages in a trusted network. Direct is secure, easy-to-use, inexpensive, and approved for use by nationally recognized experts and organizations. Direct utilizes identity-proofing to ensure messages are only accessible to the intended recipient, per the protection regulations of the Health Insurance Portability and Accountability Act (HIPAA).

CyncHealth’s DSM product provides a HISP Services solution with 2 options:

  • Webport (aka Webmail) solution that is web based and can be accessed by users who have gone through the process of creating an account and reviewed the training. This option is provided as part of participation with CyncHealth at no additional cost.
  • DirectXDR solution that integrates DSM with the Electronic Health Record (EHR) so that DSM can be sent and received as the user is in the EHR. This is available to CyncHealth participants at an additional annual cost and can be setup usually within approximately 2 weeks.
