Clinical Portal
The Clinical Portal is the state- designated Health Information exchange. It puts the flow in workflow, making it easier for care teams to access the data they need when they need it.

Prescription Drug Monitoring Program
Whether you need to reconcile a patient’s medication history, check fill dates from a different pharmacy, or pull reporting data for compliance, Nebraska’s PDMP saves you time and money. The PDMP is a digital database that provides healthcare providers with critical information about a person’s prescription history.

Workflow Efficiencies & integration
Our workflow integration promotes continuity of care by reducing the risk of errors or data inconsistencies during patient handoffs and care transitions.

Analytics as a Service
Streamline information sharing and enhance efficiency across the health ecosystem with Analytics as a Service. Receiving timely, accurate, complete, and consistent information improves the effectiveness of care coordination.

Extract data to conduct research, review population information, and generate visualizations with the Nebraska Healthcare Collaborative’s Research Hub! This hub is a self-service platform containing de-identified data used to facilitate research requests.

Closed Loop Referral Platform
Because health-related social needs (HRSN) information is a crucial part of patient care, we are committed to providing social care referral access and information to our participants. We partner with vendors to offer a Closed Loop Referral Platform (CLRP), connecting health and social care through a coordinated network of health and social service organizations.

Interoperability Solutions
With secure access to aggregated health information, CyncHealth participants can feel confident in the data. We are technology agnostic, focusing more on the completeness and accuracy of important health information than on the brand of technology used to collect it. We do this by ensuring interoperability regardless of Electronic Health Record (EHR) vendor, health system, or data standards.

Bed Dashboard
In healthcare, time is of the essence. Providers must be able to know the physical and staffed capacity of other facilities without having to cold-call. The Bed Dashboard is a digital tool showing hospital bed capacity, helping providers get patients the placements they need.

Direct Secure Messaging
As a Health Information Service Provider (HISP), CyncHealth provides Direct Secure Messaging utilizing the Direct Standard™, a technical standard for exchanging health information between health care entities (e.g., primary care physicians, specialists, hospitals, clinical labs) for sending messages in a trusted network.

Event notification
CyncHealth’s Event Notification Services (ENS) has what you need for better care coordination, timelier follow-ups, and reduced readmissions. Our ENS alerts are real-time notifications sent to providers and care managers to inform them of a recent admit, discharge or transfer within an Emergency Department, Inpatient setting or Skilled Nursing facility.

Public Health Reporting
CyncHealth automatically transmits relevant patient data from electronic health records to public health agencies, giving them timely information and enabling them to determine if action is needed. Reporting and tracking various infectious diseases helps public health get an idea of the health of their community. This transmission helps to alleviate reporting burden on providers, accelerates the detection of potential outbreaks, and supports more effective disease control efforts.
Clinical Portal
The Clinical Portal is the state designated Health Information Exchange. It puts the flow in workflow, making it easier for care teams to access the data they need when they need it. The platform shows a patient’s comprehensive and longitudinal health history, which includes encounter-level reports, diagnostic history, allergies, labs, immunizations, medication history, and more!
A feature within the Clinical portal is Image Exchange. Instead of just viewing the report, clinicians now have the ability to see the image associated with the report.
This service is available for providers, payers, and pharmacy participants.
Prescription Drug Monitoring Program (PDMP)
The PDMP collects prescription data from pharmacies and providers and stores in a centralized portal. Authorized providers can log into the PDMP portal to access and review a patients medication history ensuring safer prescribing practices and reducing the risk of prescription drug abuse. Along with being a standalone portal, the PDMP can be integrated into a provider’s EHR, allowing for improved compliance and streamlined workflows.
This service is available for providers and pharmacy participants.
Workflow Efficiencies and Integration
- Single Sign-On (SSO) allows users to access multiple CyncHealth products, using one unique login ID/password combination, provided the user has been provisioned to use those products.
- EHR data integration improves workflow efficiencies, providing options for PDMP information (Nebraska only) and/or Direct Secure Messages to be sent directly into the participants EHR, enabling this type of information to be viewed in the EHR versus a user having to access a separate system.

Closed Loop Referral Platform
CyncHealth also collaborates with United Way of the Midlands/2-1-1 Coordination Center to broaden the reach of the social care ecosystem in Iowa and Nebraska, supporting the belief that social care is community care, is health care.
This service is available for providers, payers, pharmacies, and ACOs.
Interoperability Solutions
Although interoperability is at the core of all CyncHealth solutions, we take special care to:
- Data specifications: We ensure shared data meets or exceeds the most current version of the CyncHealth Data Specifications. These are approved by governing entities representing all participants and include the most recently published United States Core Data for Interoperability (USCDI) data standards in effect.
- Providers using certified EHR technology send data according to USCDI standards.
- Payers send claims data according to X12 electronic data interchange standards.
- Data quality and completeness: By focusing on the data’s reliability, CyncHealth ensures a comprehensive view of health information to enhance decision-making for all participants.
- Data sharing: We have a number of ways to provide aggregated data back to participants, including, but not limited to:
- Various file formats utilizing all supported file transport protocols
- Open-source and/or custom software solutions
- Application Program Interface (APIs)
- Bi-directional interface connections

Bed Dashboard

Direct Secure Messaging
CyncHealth’s DSM product provides a HISP Services solution with 2 options:
- Web Portal is where secure messages can be accessible through a separate web application, much like using Gmail. This option is provided as part of participation with CyncHealth at no additional cost.
- DirectXDR is an optional solution that enables secure messages to be sent directly to the Electronic Health Record (EHR). This is available to CyncHealth participants at an additional annual cost and can be setup usually within approximately two weeks.

Syndromic Surveillance also helps to identify at-risk populations with real-time alerts so targeted interventions can occur. The result? More informed decisions for a healthier community.

Electronic Lab Reporting
Automatic laboratory result reporting is possible with CyncHealth! Electronic Lab Reporting (ELR) helps notify the state of any infections or reportable diseases to help curb potential outbreaks. This interoperability between hospitals, public health departments, and the Centers for Disease Control and Prevention (CDC) makes for healthier communities!
Electronic Case Reporting
CyncHealth, in partnership with the Nebraska Department of Health and Human Services and the CDC can submit E-case reporting (ECR) for 208 reportable conditions to local public health departments across the state of Nebraska on your behalf. Eliminating the burden on providers, this near real time reporting capability will allow Public Health officials to conduct contract tracing, offer services, or provide other public health actions as needed.
Immunizations
CyncHealth Nebraska is here to assist with immunization reporting to help you meet promoting interoperability requirements! To accomplish this, we’ll get your Vaccination Update feed (VXU) set up, which is the electronic submission of immunizations to the Nebraska State Immunization Information System (NESIIS) registry. As an added bonus, getting this feed set up also helps you meet data completeness requirements with CyncHealth. (See Interoperability Solutions above).
Our team also assists with Query by Parameter (QBP), which is a query request to NESIIS to request historical immunization details. This can then be added to the facility’s EHR in the patient’s documentation.
