Redesigned Patient Health Data Tool Streamlines Access Between Clinics in Iowa
The Provider Portal integrates various healthcare information systems to give providers a more comprehensive picture of patient health, supporting patient-centered care coordination between clinics, hospitals and other care sites.

DES MOINES, Iowa, Jan. 29, 2025 — CyncHealth Iowa, the health information exchange for Iowa and a 501(c)(3) nonprofit, has announced a new tool that streamlines access to near-real-time patient health data for healthcare providers and payers. Now available for all CyncHealth Iowa participants, the Provider Portal is a solution designed to simplify access to health information records for authorized users. The tool is part of CyncHealth Iowa’s mission to advance interoperability —the ability for different digital healthcare systems to share information securely — and improve patient outcomes across the Midwest.

“The Provider Portal allows caregivers to see the most complete picture of a patient’s health,” said CyncHealth Iowa Board Chair and Montgomery County Memorial Hospital Chief Executive Officer (CEO), Ron Kloewer, MHA. “In today’s digital world, patients expect that their physicians and care teams have all the information at their fingertips. Electronic medical record systems are not created equal, and information is too often missing, which wastes valuable time. The CyncHealth Provider Portal fills the gap.”

The Provider Portal integrates various healthcare information systems to give providers a more comprehensive picture of patient health, supporting patient-centered care coordination between clinics, hospitals and other care sites. When providers have longitudinal data at their fingertips, care coordination becomes easier — leading to more efficient care and a potential reduction in patient readmission. More efficient healthcare systems can lighten the load for providers by saving time and reducing duplicated efforts. As a result of increased efficiency, healthcare systems can save money by avoiding repeated testing.

Once logged into the system, a user-friendly interface allows providers to see in-depth patient information over time. Records include demographics, vitals, conditions, labs, allergies, and more. Additional updates are planned to further simplify user experience and provider collaboration in the portal.

“We developed this tool with Iowa providers in mind, giving them a more complete clinical profile of the patients they care for,” said CyncHealth Iowa Board Member and University of Iowa Health Care Chief Information Officer (CIO) Lee Carmen, BSEE. “As a result, patients receive better care.”

Payer participants, like Medicare, Medicaid, and commercial insurance providers, can also leverage data from the Provider Portal. This offers access to high-quality longitudinal health records for their patient population, providing actionable data for informed care decisions, timely interventions, and preventive care.

The Provider Portal was created using standards set by the United States Core Data for Interoperability (USCDI), a set of health data classes and constituent data elements for nationwide, interoperable health information exchange.
